What is CFSS?
The Community First Services and Supports (CFSS) program is Minnesota's home and community-based service program that replaced the Personal Care Assistance (PCA) program. CFSS provides support to individuals of all ages who need help with daily activities due to a disability or chronic condition.
Requirements
Who is Eligible?
To qualify for CFSS, you must meet the following requirements:
Medical Assistance Enrollment
Be enrolled in Medical Assistance (MA), the Alternative Care (AC) program, or specific types of MinnesotaCare.
Health Assessment
Complete a formal health assessment through your local county, tribal nation, or managed care organization.
Demonstrated Need
The assessment must determine a need for daily physical care or behavioral intervention support.

How to Get Started
Getting started with Care Planners Inc. is a simple 4-step process.
Check Your Eligibility

Complete a Health Assessment
Contact your local county, tribal nation, or managed care organization to schedule your formal health assessment.
Choose Care Planners Inc.
Select Care Planners Inc. as your CFSS Agency Model provider. We'll handle all the paperwork and setup.
Begin Your Care
We hire, train, and pay your support workers. Your care begins — and your independence is supported.
What is the Agency Model?
Under the Agency Model, Care Planners Inc. acts as the employer of your support workers. This means we handle all hiring, training, scheduling, payroll, and compliance — so you receive consistent, high-quality care without the administrative burden.
